Patient Medical Home/Primary Care Network Coordinator (PMH/PCN)

Job Summary: The PMH/PCN Coordinator (Patient Medical Home/Primary Care Network) reports to the Executive Director and is responsible for working with primary care practices to support foundational work on Patient Medical Homes (PMH) and Primary Care Networks (PCN).

The PMH/PCN Coordinator maintains accountability for processes involving activities that support the PMH, attachment and the HCR (Health Connect Registry) and the NPDoFP’s (North Peace Division of Family Practice) Emergency Management plan. Development of local and regional recruitment & retention strategies along with participation in recruitment & retention activities.

This is a permanent full-time position.

Key Responsibilities & Duties:

Attachment

· Maintain and manage HCR.

· Creating a process or system to keep accurate, up-to-date primary care provider information, including attachment capacity.

· Maintain referral pathways for unattached patients presenting in emergency/acute care.

Emergency Planning

· Update the internal organizational EM plan, incident command and emergency operations center structures, that includes local health authority input for role clarity and coordination, member engagement, communication, and mobilization.

Patient Medical Home

  • Coordination of PMH networks (e.g., unassigned inpatient care, prenatal, REEF schedules and invoices)
  • Support FP cultural safety and competency training.
  • Support active involvement of physicians and physician leaders in local development and implementation of PMH AND PCN.

o Support the increased utilization of interprofessional teams in private practice.

Recruitment & Retention

· Development of local and regional recruitment strategies along with participation in recruitment activities.

  • Manage and maintain locum house.
  • Organization of, and attendance at, member engagement activities and events, including physician site visits.
  • Developing physician integration and retention road maps, tool kits, and processes.
  • Compiling community integration services and resources that are available in the community.
  • Welcoming and integrating new FPs into community.
  • Supporting and connecting new FPs and their families to the services and programs that are available in the community.
  • Facilitating collaboration and communication between FPs, the health care system and community partners.
  • Working in partnerships with local community groups and regional organizations to foster discussions and solutions around physician integration and retention.

Qualifications

Education, Training and Experience

· Any health-related degree or diploma or equivalent job experience i.e., advanced MOA.

· Excellent interpersonal, communication and assessment skills.

  • Ability to work independently and in collaboration with others.
  • Highly proficient in the Microsoft office suite.
  • Utilizes initiative, critical thinking and creative problem-solving ability to develop and implement plans.
  • Excellent interpersonal and relationship building skills with ability to liaise with physicians and stakeholders in a professional manner.
  • Ability to work flexible hours including occasional evenings or weekends.
  • Able to write detailed reports, executive summaries, policy documents, and other communications.
  • Seeks increased responsibility while remaining conscientious, thorough, accurate, and reliable with respect to the organizational goals and the needs of the members and staff. This includes being available and responsive to issues and concerns as they arise.
  • Able to exercise discretion, tact, and diplomacy in dealing with highly sensitive and confidential matters.

· Other experience with: primary health care, family Physicians, health care teams would be an asset.

What we offer

· Employer retirement contributions after 6 months.

· $30 per hour based on education and experience, prorated vacation of 3 weeks per annum, prorated personal time of 5 days.
