Position Description
POSITION SUMMARY:
Central Access is an exciting new service at St. Joseph's in the Mental Health and Addiction Program. The service will be responsible for all intake functions and navigation of outpatient referrals. The role of the Occupational Therapist within Central Access will be to plan, implement, and evaluate client-centered occupational therapy services consistent with the mission and values of St. Joseph's Healthcare. The Occupational Therapist assesses and diagnoses physical, emotional, social and cognitive capacity and performance, occupational demands and environmental factors and interventions to enable participation in activities of everyday life to promote and optimize health and well-being of clients consistent with the scope of practice of occupational therapy. Key functions of the Occupational Therapist encompass clinical, professional and non-clinical activities as outlined in the Profile of Occupational Therapists in Canada that are shaped by their program needs and client population. All tasks are performed in ways consistent with the competencies of the College of Occupational Therapists of Ontario (COTO) including; assuming professional responsibility, thinking critically, demonstrating practice knowledge, utilizing occupational therapy process to enable occupation, communicating and collaborating effectively, engaging in professional development, managing practice and advocating within systems. Given that St. Joseph's Healthcare is affiliated with McMaster University and Mohawk College, the Occupational Therapist is expected to engage in appropriate teaching and research activities.
- Bachelors Degree in Occupational Therapy
- Masters degree in Occupational Therapy, preferred
- 1 year related clinical experience
- Maintains current working knowledge of relevant legislation which includes the following: Mental Health Act; Health Care Consent Act; substitute Decisions Act; public Hospitals Act; Long Term Care Act; Child and Family Services Act; PHIPA; and Excellent Care for All Act, preferred
- Regulated Health Professional, Registration with the College of Occupational Therapists of Ontario
- Membership in the Ontario Society of Occupational Therapists and/or membership in the Canadian Association of Occupational Therapists
- intake experience in mental health and addictions required

- Demonstrated knowledge and experience working with the various populations served.
- Demonstrated knowledge, sensitivity and expertise in the care of youth, seniors and their families.
- Demonstrates superior knowledge in engaging with patients and/or families who may be reluctant to seek help and establish rapport in various scenarios.
- Gentle Persuasive Approach or Motivational Interview training is preferred.
- Proven ability to perform comprehensive bio-psychosocial mental health and addiction screening and assessments, including crisis assessment and interventions, as well as the ability to apply research and evidence to inform processes and program development.
- Ability to ensure continuity of care between services and development of coordinated care plans to promote community integration for service users.
- Able to work under pressure, prioritize workload and meet deadlines.
- Proven ability to work as an effective member of an inter-professional team with exceptional interpersonal, advocacy, negotiation and conflict resolution skills both with internal and external partners
- Willingness and ability to continue to improve professional competence and knowledge.
- Exceptional analytical thinking and creative problem solving skills.
- Proficient computer skills for report writing, assessments, word processing, workload measurement and access to information, etc.
- Manages caseload in the areas of assignment by acknowledging and consulting regarding the need for occupational therapy involvement and seeks out referrals to ensure comprehensive client care
- Screens referrals by gathering preliminary information from the referral itself, the health record and the team of care providers and using the gathered information to determine if the client requires Occupational Therapy services and if so, determine the client’s level of priority based on the discipline priority process
- Completes an appropriate depth of occupational therapy assessment as required by COTO Assessment Standards by reviewing the client acuity, client goals, and care requirements based on best practice protocols or care paths and discharge date. Compiles subjective and objective data from informal and formal sources regarding client’s desired or expected occupational performance (e.g., self-care, productivity and leisure), the client’s performance components (e.g., cognitive, affective, perceptual, spiritual, social and physical) and environments (e.g., cultural, social, institutional and physical affecting performance
- Analyzes the data to determine the client’s strengths and weaknesses and occupational performance issues and develops the occupational therapy client-centred plan of care that is congruent with client and family goals, best practice protocols or care paths and the practice setting
- Assigns the collection of measurements or data to Occupational Therapy Assistants where available following the COTO Standards for Assigning Service Components to Non-Registrants to use in the assessment process.
- Works collaboratively with members of the interdisciplinary team to identify and prioritize an individuals needs, assess and respond to risk, and identify and optimize care options including inpatient and outpatient services, general practitioners, and a range of community services
- Determines and completes treatment interventions that are consistent with the practice setting, the discipline priorities, congruent with evidence based practice and clinical experience, and the caseload demands. These interventions may be provided to individuals or groups and include, but are not limited to the following: (1) Therapeutic Activity Programs (Enhance Physical, Social, Emotional, Cognitive, Perceptual Function); (2) Basic ADL Training Programs; (3) Instrumental ADL Training and Life Skills Training Programs; (4) Productivity Interventions to Engage in Employment, Education or Volunteering; (5) Adapted Methods/Activity Modification/Recommendations; (6) Orthotic/Splinting Prescription; (7) Wheelchair/Seating/Mobility Prescription; (8) Client & Caregiver Education/Psycho education; (9) Assistive/Safety Devices Prescription; (10) Community/Activity Reintegration; (11) Environmental Adaptation; (12) Skin Protection/Bed Positioning; (13) Supportive Counselling/Psychotherapy; (14) Manual Therapy & Modalities
- Assigns the completion of therapeutic intervention to Occupational Therapy Assistants where available following the COTO Standards for Assigning of Service Components to Non-Registrants and clearly documenting the scope of the work assigned
- Advocates for the client with appropriate consent with community agencies, funding agencies, school boards, colleges and universities and workplaces to facilitate meeting client’s goals
- Re-evaluates, monitors and adjusts the client-centred plan of care based on the up-to-date analysis of the data and attainment of client goals
- Completes accurate workload statistics within expected timelines and ensures work activity is in alignment with the current benchmarking activities of the organization
- Documents all elements of the clinical process in the health record adhering to COTO, facility and discipline standards
- Completes other program data collection to support ministry requirements such as RAI data, FIM scores and MDS data consistent with the expertise of the discipline
- Contributes to the process of discharge planning which could include communicating assessment results, providing, providing recommendations re: level of care need, restrictions, safety, the interventions provided, the coordination of services, the completion of the referral and the communication process with involved care providers
- Provides support and education to patients and their families affected by mental illness and/or addictions
- Participates in activities that enhance the quality of care at St. Joseph’s Healthcare Hamilton including but not limited to team meetings, department meetings, quality teams, unit/program based meetings/committees, accreditation teams and corporate committees relevant to the occupational therapy role, expertise and scope of practice (i.e., falls, accessibility, pressure ulcer audits, anti-stigma, etc.)
- Participates in appropriate clinical research investigations for the purpose of improving the quality of Occupational Therapy care and inter-professional care as appropriate.
